For your first visit:

  • Insurance card
  • Medical history
    (Can be downloaded below. You are also welcome to fill it out at the office)
  • Current x-rays of the teeth, if available
    (not older than 1 year)
  • Allergy certificate
  • Drug list

You are welcome to print out the anamnesis questionnaire, fill it out at home and bring it with you or fill it out at the office.
